What is SUDEP?

SUDEP (Sudden Unexpected Death in Epilepsy) refers to the sudden and unexplained death of a person with epilepsy, with no other identifiable cause found. While rare, it affects more than 1 in 1,000 people with epilepsy each year and is a leading cause of death in individuals whose seizures are not fully controlled.

SUDEP often occurs during sleep, and the person is usually found in bed, sometimes in a face-down position. In some cases, there is evidence of a seizure happening close to the time of death, though not always. The exact cause remains unclear, and it may vary between cases. Some research suggests that seizures could interfere with heart rhythm, while other studies point to breathing difficulties or changes in brain function following a seizure.

What Causes SUDEP?

The exact cause of SUDEP is still unknown, though several potential factors are being studied. SUDEP often happens during sleep, which means the circumstances are usually not witnessed, leaving some questions unanswered.

Researchers are looking into several possibilities, including:

  • Breathing problems: Seizures may cause brief pauses in breathing (apnea). If breathing doesn't resume quickly, it can reduce the oxygen supply to the brain and heart, which can be life-threatening.
  • Heart rhythm: In rare cases, seizures may trigger irregular heartbeats or even cardiac arrest.
  • Brain function: Some seizures may affect parts of the brain that control vital functions like breathing and heart rate. Disruptions in these areas could contribute to SUDEP.

It’s also possible that SUDEP results from a combination of these factors or from mechanisms we don't yet fully understand.

Can SUDEP Be Prevented?

While there is no guaranteed way to prevent SUDEP, one of the most important steps is to reduce seizure frequency. Research suggests that people who experience convulsive seizures may be at higher risk, but everyone with epilepsy can benefit from better seizure management.

Here are some ways to reduce the risk of SUDEP:

  1. Improve Seizure Control:

    • Take medications as prescribed and consult your healthcare provider regularly, especially if seizures continue to happen.
    • Explore additional treatment options, such as epilepsy surgery, neurostimulation devices, or dietary therapies, if medications aren’t enough.
  2. Prioritize Well-being:

    • Maintain a healthy lifestyle by getting enough sleep, eating well, managing stress, and avoiding known seizure triggers such as missed medications or alcohol use.
  3. Monitor Heart Health:

    • Discuss with your doctor the possibility of a heart evaluation, particularly if your epilepsy diagnosis is unclear or your seizures are difficult to control.
  4. Practice Seizure Safety:

    • Make sure that family members and caregivers know seizure first-aid and take extra precautions around water and other situations where seizures could pose an added risk.

Taking proactive steps to manage epilepsy can help reduce the risk of SUDEP, and working closely with your healthcare team can provide support and guidance in navigating the challenges that come with epilepsy.
